Overview
Breezing is a metabolic testing device, developed out of Mayo Clinic-affiliated research, that measures a patient’s Resting Energy Expenditure (REE) through a 10-minute breath test. Breezing Pro is its HIPAA-compliant companion app, connecting via Bluetooth to run tests and securely store clinical results. Clinicians previously relied on REE estimates from generalized formulas based on age, height, weight, and activity level, estimates that could miss a patient’s actual measured REE by a wide margin.
Goals
- 1.Match device behavior exactly: mirror the Breezing Pro hardware's measurement logic precisely, with no room for drift between what the device reads and what the app shows.
- 2.Design for speed under pressure: keep setup, testing, and review fast enough to survive real clinical time constraints.
- 3.Build in compliance from the start: meet HIPAA and FDA requirements on every screen that touches patient data, including a local-only storage option for partners who required it.
- 4.Make failure states impossible to miss: a hardware-connected app has to be invisible when it works and unmistakably clear the moment it doesn't.

The Problem
- Clinicians relied on REE estimates from generalized formulas based on age, height, weight, and activity level
- Those formulas could miss a patient’s actual measured REE by a wide margin, in one real reading, by 18%
- No individualized way to account for a patient’s own metabolic variation
- Clinical workflows left little room for setup, testing, and review to be anything but fast and reliable
- HIPAA and FDA compliance requirements applied to every screen handling patient data
Problem to solve
The app had to be invisible when it worked, and unmistakably clear the moment it didn’t.
Strategy & Execution
Constraints shaped every decision: HIPAA and FDA compliance, reliable BLE communication to the device, measurement logic that mirrored the hardware’s behavior exactly, and local-only storage for the partners who required it. The guiding principle was clarity under pressure: streamlined flows, clear system feedback, minimal cognitive load, explicit state handling, and interfaces built for speed.
Here’s a full test, start to finish, from adding a new patient to the ten-minute countdown that measures them.
Starting from an empty patient list: nothing to test until a patient is added.
Adding a new patient: name, sex, date of birth, height, weight, and lifestyle.
Lifestyle uses a simple, unambiguous set of options instead of a free-text field.
Before the test starts, the patient verifies their own information is correct.
Turning on the Breezing Pro device.
Pairing over Bluetooth.
Scanning the QR code on the sensor cartridge package.
Inserting the cartridge: once opened, the test has to start within 5 minutes.
Calibrating the device on a flat surface before it touches the patient.
Attaching the mask and securing the device to the patient.
A ten-minute countdown while the patient breathes normally, with Stop Test always visible.

Results had to stand next to the old estimate and make the difference obvious, then fold into a history a clinician could track across visits.
